This is the working doc for collecting every answer needed for Coa’s 8(a) annual review submission in MySBACertifications. Fill in the Answer column for each row, draft any free-text fields offline, then a separate session will fill all of it into the portal in one pass.
How to use this file
For Ari + Phillip: Walk through each section below, fill in the _ placeholder in each Answer column. For free-text questions, draft directly in the cell (or in a scratch doc next to it). For dropdowns where options aren’t captured, decide which option fits and note it. Section 17 is N/A and already auto-completed. Section 3 (economic disadvantage) needs Ari’s personal financials — pull from Mercury / retirement accounts / property records.

- After the questionnaire is fully filled, upload required documents in Step 2, handle spouse invite in Step 3 if applicable, then stop at Step 4 and wait for sign-off.

Step 3: Contributor Invite

- Invite Spouse button — required only if Ari is married (spousal financials feed economic disadvantage analysis).
- Owners table: Phillip Jo, phillip@coa.solutions, status Unsent. Per the form: since Ari owns 80% (>51%), Phillip does NOT need to submit a separate questionnaire. Leave Unsent.
Step 4: Sign

Blockers: Submit button is disabled until (a) all questionnaire sections complete and (b) all required documents uploaded with document types selected.
Attestation includes (read full version in file):
- Information is true and correct (criminal liability under 18 USC 1001)
- All required change requests submitted to SBA; all other changes notified within 30 days
- Awareness of penalties: up to $500K fines + 10 years imprisonment + treble damages under False Claims Act + double damages under Program Fraud Civil Remedies Act + debarment + program termination
Status: NOT signed, NOT submitted. Ari approval required before any click on Submit.
Working sequence
- Walk this file with Phillip, fill in each
_with the answer. - For free-text questions: draft offline (in this repo), paste into the form.
- For dropdown questions where options weren’t captured (2.1, 5.2, 9.4, 11.3, 6.6 NAICS): open the dropdown on screen, select per Coa’s situation.
- Sections 12 (SWOT), 5.1 (executive summary), 11 (marketing) need substantive drafting — they’re the most narrative-heavy.
- Section 3 (economic disadvantage) requires Ari’s personal financial data — pull from Mercury, retirement accounts, real estate holdings.
- Section 9.1 — confirm Mentor Protégé Agreement status (CoCoa JV with Coforma is in formation; Phillip is leading the Apr 2026 Virginia SCC re-filing).
- Section 16 — assemble owner/affiliate payment list from Gusto + Mercury for last program year.
- After all questionnaire fields done, upload required docs in Step 2.
- Then Step 3 (spouse invite if applicable, leave Phillip Unsent).
- Then Step 4 Sign — Ari reviews everything, then explicit per-action approval before clicking Submit.
